Objective: To evaluate feasibility of the guidelines of the Groupe Francophone de Réanimation et Urgence Pédiatriques for limitation of treatments in the paediatric intensive care unit .Design: A 2-year prospective survey.Setting: A 12-bed PICU at the Hôpital Jeanne de Flandre, Lille, France.Patients: Were included when limitation of treatments was expected.Results: Of 967 children admitted, 55 were included with a 2-day median delay. They were younger than others , had a higher paediatric risk of mortality score , and a higher paediatric (...) overall performance category score at admission ; all p<0.002. 34 children died. A limitation decision was made without meeting for 7 children who died: 6 received do-not-resuscitate orders and 1 received withholding decision. Decision-making meetings were organised for 31 children, and the following decisions were made: 12 DNROs , 4 withholding , with 14 withdrawing and 1 continuing treatment . After limitation, 21 children died and 10 survived . 13 procedures were interrupted because of death and 11 because of clinical improvement . Parents’ opinions were obtained after 4 family conferences , 3 days after inclusion. The first meeting was planned for 6 days after inclusion and held on the 7th day after inclusion; 80% of parents were immediately informed of the decision, which was implemented after half a day.Conclusions: GFRUPs procedure was applicable in most cases. The main difficulties were anticipating the correct date for the meeting and involving nurses in the procedure. Children for whom the procedure was interrupted because of clinical improvement and who survived in poor condition without a formal decision pointed out the need for medical criteria for questioning, which should systematically lead to a formal decision-making process. A successful attempt to bring all of Freud's discussions of the concepts of repression and defense into systematic form. Madison also argues that there is an observational language which corresponds to- Freud's theoretical language; by translating these concepts into observational terms, we can bring Freudian psychology "up to date."--S. The treatment decisions of competent adults, especially treatment refusals, are generally respected. In the case of minors something turns on their age, and older minors ought increasingly to make their own decisions. On the other hand, parents decide on behalf of infants and young children. Their right to do so can best be justified in terms of the importance of preserving intimate family relationships, rather than in terms of the child's best interests, although the child's best interests will most often (...) follow from this arrangement. Nevertheless, there are and ought to be legal, ethical, and financial constraints on parental decision making. (shrink)

This work is primarily an interpretation of Indian Logic preserved in China.

The book offers an introduction to the moral concepts and value of health care. It is written by a moral philosopher, a doctor and a nurse and contains questions, cases and exercises which are suitable for medical, nursing and all students and commentators on health care. Moral dilemmas include consent, confidentiality, the giving or withholding of information, and the economics of health care. The issues of artificial reproduction, terminal care and the research and testing of drugs are addressed.

Examining the health of the population of the industrial world, the authors conclude that it is no healthier than it used to be, rather that diseases have been substituted, not eliminated. They suggest an alternative approach to health care which derives from a philosophy of nature.

This selection of papers that were presented (or nearly so!) to the Boston Colloquium for the Philosophy of Science during the seventies fairly re presents some of the most disturbing issues of scientific knowledge in these years. To the distant observer, it may seem that the defense of rational standards, objective reference, methodical self-correction, even the distin guishing of the foolish from the sensible and the truth-seeking from the ideological, has nearly collapsed. In fact, the defense may be seen to (...) have shifted; the knowledge business came under scrutiny decades ago and, indeed, from the time of Francis Bacon and even far earlier, the practicality of the discovery of knowledge was either hailed or lamented. So the defense may be founded on the premise that science may yet be liberating. In that case, the analysis of philosophical issues expands to embrace issues of social interest and social function, of instrumentality and arbitrary perspective, of biological constraints (upon knowledge as well as upon the species-wide behavior of human beings in other relationships too), of distortions due to explanatory metaphors and imposed categories, and of radical comparisons among the perspectives of different civilizations. Some of our contributors are frankly programmatic, showing how problems must be formulated afresh, how evasions must be identified and omissions rectified, but they do not reach their own completion.
